It's just a lot of trial and error. I've got a couple wonderful studio lights now, but I spent years having to try to light models somewhat evening with a ton of house lamps. And my first attempt at using candles happened because my power went out during a shoot. A lot of my more interesting ideas just stem from situations where the lighting options were awful and I was just desperately trying to get enough light onto my model such that a decent image would result.
